These pages contain the necessary forms and instructions
for applying for a Florida motor vehicle title by mail.

**All forms must be completed in dark blue or black ink or they will be returned.**

Please read each item carefully. It will take you through your package step-by-step. All incomplete applications will be returned to the sender.

  1. NEW VEHICLES purchased through a dealership must submit the ORIGINAL Manufacturer's Certificate of Origin properly assigned by the selling dealer (copies will not be accepted).
    USED VEHICLES purchased through a dealership OR private individual must submit the ORIGINAL title properly signed by the seller and purchaser. If the vehicle is already titled in your name you must submit the original title. If the title is being held by your lien holder we will write a letter requesting they release it to our office. Please provide us with their address, their FEID number and your account number.
  2. For all vehicles purchased from a dealer a separate odometer statement must accompany the application. Each owner must SIGN and PRINT their name on all reassignment forms and odometer statements.
  3. For all vehicles purchased from a dealer the sales invoice from the selling dealer must accompany the application, legible copy is acceptable.
  4. A copy of the Driver's License for each owner must accompany the application.
  5. If you are a Military member claiming Florida as your home of record at the time of your enlistment, you may be exempted from the $100.00 initial registration fee. Please complete form HSMV82002 and submit a current copy of your military orders and driver's license.

    Please note: sections 7,10,11 and 13 may not apply and may be omitted

    SECTION 1: Owner/applicant identification must be accurately completed showing the complete name, address, sex, date of birth and the FL driver's license number of the owner's and/or lessee if applicable. PLEASE NOTE: a Florida street address is MANDATORY and must be given in order to title your vehicle in this state. This section also provides a space to indicate if the names should be joined by "AND" or by "OR" in the case of joint ownership. If neither box is checked the title will be issued with "AND".

    If you are leasing your vehicle the lease company would be listed as the owner and you would be listed as the lessee. You will need to submit a properly executed ORIGINAL Power of Attorney (copies will not be accepted), the lease company's Florida Sales Tax Number and the lease company's FEID number.

    SECTION 2: Vehicle description must be completed in full showing all required information.
    Weight is required for all cars, vans, trucks and motorhomes.
    Length is required for travel trailers and campers.
    BHP/CC is required for motorcycles.
    COLOR is required for all card, vans, trucks, motor homes and motorcycles.

    SECTION 3: Vehicle Status and Usage must be completed indicating how the vehicle will be used.

    SECTION 4: If applicable, your Lienholder information must be completed giving the lienholder's name, address, date of lien and their FEID number.

    SECTION 5: Transfer Type must be completed indicating how the vehicle was acquired, the date of lien and if the vehicle is new or used.

    SECTION 6: Odometer Declaration must be completed to show the current odometer reading, date read and status. BE SURE TO READ THE ODOMETER DISCLOSURE STATEMENT CAREFULLY, ANY ALTERATIONS WILL VOID THIS FORM.

    SECTION 8: This section applies to previously titled vehicles only (vehicles with a MCO do NOT need to be verified). NO CROSSOUTS, ERASURES OR ALTERATIONS WILL BE ACCEPTED. A Law Enforcement Officer or a licensed dealer must physically verify the vehicle's ID number. The ID number must be identical to the number shown on this form. If being verified by an officer, the date, officer's signature, printed name and badge or ID number must be completed. If the VIN is being verified by an out of state motor vehicle dealer, the verification must be submitted on their letterhead stationery.
    Military Personnel Only: A Provost Marshal (an officer who supervised the military police of command) or a commissioned officer in active military service with a rank of 2nd lieutenant or higher or an LNC, "Legalman, Chief Petty Officer, E-7" may verify the odometer and VIN number on your vehicle.

    SECTION 12: Application Attestment & Signatures must be signed by the applicant and co-applicant, if applicable.

    Florida State Sales Tax is 6% of the purchase price with trade allowance given. REBATES ARE NOT DISCOUNTS AND CANNOT BE DEDUCTED FROM THE TAX LIABILITY. Some individuals may be exempt from paying sales tax. Please see the Sales Tax Computation Chart for detials.

    The State of Florida requires all motor vehicles with Florida tags to have a minimum of $10,000 Personal Injury Protection and a minimum of $10,000 Property Damage Liability Insurance in force at all times. To meet the Florida insurance requirements you may either purchase insurance from an agent located and licensed in the state of Florida or from an insurance company which is in compliance with the Florida statute. To be in compliance an insurance company issuing insurance for Florida registered vehicles must issue the policy through or be countersigned by a resident agent in Florida. The agent must report the issuance of the policy or any lapses to the Florida Department of Hwy Safety and Motor Vehicles in a "format compatible with the data processing capabilities" of the department. You should also understand as long as you have Florida tags on your vehicle this insurance must remain in effect. Any insurance cancellation will result in suspension of your driver's license and tag registration. If your present insurance has been written outside of Florida you must have your policy countersigned by a resident agent in Florida.

    When transferring a license plate PLEASE INCLUDE THE REGISTRATION OR A LEGIBLE COPY OF THE REGISTRATION TO THE LICENSE PLATE.
    To transfer a license plate the vehicle the tag is coming off of must be sold or placed in dead storage or the $100.00 Initial Registration Fee must be paid. Proof of vehicle transfer may include the following: invoice from selling dealership showing the vehicle was used as a trade OR an affidavit stating the new owner's name and the date of sale (you may use the enclosed Disposal Affidavit).

    Your remittance should be in the proper amount as indicated on the calculations of fees list made payable to Rod Northcutt Tax Collector. Upon the receipt of the properly completed application, sufficient proof of ownership, federal odometer statement, bill of sale, proof of Florida PIP and PDL insurance coverage and the proper remittance we will promptly issue a Florida license plate and registration. All documents must be completed in dark blue or black ink. ALL INCOMPLETE APPLICATIONS WILL BE RETURNED TO THE SENDER. The Florida title will be sent to the owner (or the lien holder) once it has been issued by our state office in Tallahassee.
